A practical guide to requesting kidney care, gathering your records and knowing what happens next at AspireMed.
01 · Choose your visitFocused concern or a broader kidney review?
$199 focused nephrology telehealth: Evaluation of one defined kidney concern, abnormal laboratory result, blood-pressure question, electrolyte issue or kidney-related medication concern.
$299 comprehensive kidney consultation: Extended evaluation for complex kidney concerns, advanced chronic kidney disease, difficult-to-control blood pressure, dialysis planning or detailed review of available records. Extensive record review may require a separate estimate.
The $99 focused same-day illness visit is a separate service. Ask the practice which consultation fits your concern, and confirm the price for the selected format before booking.
Pricing and insurance details →02 · Gather your informationA short preparation checklist
- Recent and earlier blood and urine results, including creatinine and eGFR when available
- Current medicines and supplements, with doses, plus allergies
- Dated home blood-pressure readings, if available
- Relevant kidney imaging reports, specialist notes and hospital discharge papers
- Your main questions and the names of your usual clinicians
Send records through the secure intake workflow provided by the practice. If records are missing, contact us before the visit to ask what is needed; do not delay urgent medical care while gathering paperwork.
03 · Confirm the arrangementsRegistration, payment and location
You may contact AspireMed directly; the practice does not require a referral. Your insurance may require a referral or authorization. Participation, eligibility and required approvals must be checked; verification does not guarantee coverage or payment.
Required registration and consent forms must be completed before the visit begins. For telehealth, be physically located in Florida or New Mexico, have your device ready and choose a private place for the discussion.
In-person visits take place at 6051 W Commercial Blvd, Tamarac, FL 33319. Confirm your appointment before traveling.

Your physician reviews the reason for the visit, relevant history, available results, medicines and questions. An office examination, additional testing or another level of care may be needed when telehealth cannot adequately address the concern.
Before finishing, ask which results are still needed, how results will be communicated, whether follow-up is recommended and what information to prepare for it. A consultation does not guarantee a particular diagnosis, prescription or treatment.
Dialysis education and planning are available; dialysis treatments and transplant evaluations take place through the appropriate dialysis programs and transplant centers.
